A Review on Jets

Hey fellow NICU nurses,

I've been primarying a trach baby for about 6 months who will be heading home soon. We are about to get a new bunch of new nurses so they will be orienting with her (with other preceptors) since she is our only trach baby. So I was going to request some jets because I've had just about everything paired with her except a jet.

Please oh please oh please. I need a quick review course on jets to make sure I've got my head on straight. They will be one-to-one assignments for me (since someone else will have my primary patient). My trach baby has been on the vent all this time (on MMV mode), so vents are NOT the issues. Neither are micropreemies or the sickest of the sickest (b/c i've been through it all with my primary).

But I need a run through on the positioning of everything inside the isolette, which tube points up (not referring to the ETT), backup rates to help recover from bad spells, understanding all the numbers again, etc (servo pressure, etc.). I'm sure I'll be great once I jump back in there, but it's been a while!

Interested in this info as well. Our NICU educator recently sent an email asking us which procedures we'd like to have further training on, and I immediately asked her for vent training. Hope she can come up with something.

I tried to click on the "Bunnell Clinical Directory" in the post by chare above but my computer would not open the page. I came here because my e-newsletter from the Academy of Neonatal Nursing which arrived today mentioned this same website as having, "...HFJV tutorial with clinical answers and questions..." I went to their link and found some really good stuff; some rather clinical but all good even if a bit dry. Hope it helps.

Another possible resource that looks like a very good one is from the National Association of Neonatal Nurses. Their neonatal nurse practitioners put together a "Resource Guide for the Use of High-Frequency Jet and Oscillatory Ventilation". It comes as a 16-page desk reference in a 9x6 inch format which suggests strategies to use when managing infants on either HFJV or HFOV (vent settings, respiratory and blood gas abnormalities, guidelines for initiating and weaning these vents, etc.) It can hang on the vent as well by an o-ring. There is also a 3x5 inch card which is a quick-start guide you can keep in your pocket or hang on your badge holder. Both of the sizes come laminated. There is also a bibliography included for reference and further reading. you can go to http://www.nann.org to find out more.

After about 30 min with my jet patient, it all came back to me...yesterday, i was given a patient on an oscillator, so i must have been doing things right!
