A2400C for early planned discharge

Coding question:

For Medicare Part A covered patients who elect to leave prior the end of their skilled services (such as Restorative PT/OT) and NOMNC was not issued since patient would still have been covered, what date do you put for A2400C, end date of most recent Medicare stay?

Should it be ongoing?
