    PT with Fournier's gangrene of the penis and scrotum. Area was debrided-talk about slice and dice- and we did wet to dry dsgs q shift. Terrible smell, terrible wound, and he would have bladder spasms and urinate into all of this mess even though he had a foley.

    I'm a psych nurse now but had some memories of when I was a paramedic (who by the way are the only people I know that can scrape a guy's intestines off the road and return back to base for the remainder of their spaghetti dinner without missing a beat)
    I once had to make a pronouncement on a fellow who was drag racing an 18 wheeler when he hit a bridge abutment and his cab became an accordian that burst into flames. He was a bit crisp and it appeared that all his organs were shoved up into his upper body cavity, it looked rather unreal and I was compelled to try for a pulse, of course it wasn't there...there were alot of war stories, but this poor soul has stood out.

    I can't claim that my story is the worst of these by any means, but...When I graduated from my OB/GYN NP program, my first job was in a county hospital where we saw LOTS of indigent patients, homeless people, etc. I had a patient who was a new OB patient, 12 weeks pregnant, no complaints, just beginning prenatal care. I had her up in stirrups doing her pap and STD tests, concentrating on looking for signs of STD's when I noticed some movement around her rectum. A WORM crawled out of her rectum! I kid you not, I thought I was going to scream! I slowly grabbed ahold of the worm, pulled it the rest of the way out, and told her that we needed to treat some things... YUK! And I could go on about some nasty Bartholin's gland cysts, but I'll leave the pus to your imagination. Some of the other descriptions of pus in this thread have said all that needs to be said! YUM YUM! :chuckle

    OB/GYN NP that is pretty bad. How did you keep from laughing?:chuckle That reminds me of a pt I had in ED, chief complaint, " I've got chicken bones coming out of my butt" He weighed a good 350 lbs, So I went in with the doctor to retract his cheeks. Let me add, this gentleman wasn't overly fond of soap and water. The dr and I get down between his cheeks, and sure 'nuff, he had a big old fish bone that was impaled in an infected hemorrhoid. The dr and I couldn't help it we both started laughing, and so did the pt!!! :roll :roll Our next pt was a man that said a bat bit him on the neck. He was a mortiation. ED, you gotta love it.

    I have seen one pt with this. The entire lower abd, inner thighs, and penis was covered in integra (fake skin) after being debrided.

    Omg...I'm starting an ON/GYN NP program in the spring and if I ever see a worm crawl out anyone, I'm done. Changing careers. LOL

    Thanks! I am not so sure about Fornier's gangrene with this guy as the s/s don't seem to match up. I arranged a f/u appt w/ his PMD and I hope that even though he's medicaid, the doc will do this poor guy right. I have a feeling there's something going on inside his testicles like cancer or whatever....and he will probably need to have them removed. He's too non-compliant for lt therapy. Sad stuff.
