Just thought id run a recent test question that I had by you pros.

It boiled down to the following: a child comes in to the school nurse with a suspected sprained ankle. What is the primary intervention?

it was between these two:

a. assess for edema and discoloration

b. apply ice

i picked A and was wrong..we always assess first, right? if I am way off please explain it to me!

always read the questions verrry carefully. this question is asking about intervention, so everything that has to do with assessment is out. :)

I think Blue Chip is right. I think the key word there is INTERVENTION. Your intervention always follows your assessment, so the question is assuming you have completed the assessment. Very important to read the question carefully and never read too much into the question...go only on the info you are given...and if in total doubt always pick C. :)

Seems like a bad semantical dilemma. Assessment is a nursing intervention.
