Monthly report ? PA

I am from Pennsylvania and heard in a webinar recording there is a monthly report I am to be sending.... a DPH report ???? But can not find any info on such. Has anyone heard of this ? This wasn't referencing the tally sheets for the annual report but that is all I find on the web when doing a Google search. The podcast was a couple years old but I think it may be in relation to reporting diseases so it'd still be relevant. If anyone knows where I'd go to find the info I'd appreciate it !

I send a monthly report to our district office with numbers for that month. IE- number of visits, number of medications given (prn and scheduled), number of screenings done, cases of varicella/strep/conjunctivitis, ect. I have no idea what is done with the information from there.

Thank you! I spoke with one of the state's school nurses and she confirmed there is not a monthly report to send in. I was searching all over for no reason !!
