Ha! I'm used to "pleasantly" as a descriptor for confused, but I definitely see that. I tend to sugar coat things though, trying to see the best in our patients.

I've used "pleasantly psychotic" more times that I could count.

I did use "pleasantly homicidal" in report once. Patient stated he wanted to kill me but he was very nice about it and promised to talk to me first. There was a lot of Axis II in there--he had a history of trying to get reactions out of staff and it was my first time working with him--so I wasn't alarmed. But I did document it and pass in on in report to CMA.
