Nissen Fundoplication questions?

just wondering if anyone here had any experience with pd pts that have had a nissen fundoplication ?

my pt is 2 weeks post op and still having reflux, the surgeon did say that is wasn't 100% stopping reflux. the episodes aren't as severe as before surgery but are worse than they were right after.

I have worked with peds patients with a fundiplication and pretty much encountered what you have described. The procedure improved things (according to the parents since I wasn't with pt from the beginning), but did not totally take care of the problem. One of my patients had a bleed in the middle of the night and we had to take him to the ER. After several hours at the hospital he was sent home, ok. Another patient was frequently with reflux and spitting up. I just had to learn how to deal with it. Can't really offer too much in the way of advice, other than to remind you to keep their head elevated when they are down to sleep to prevent aspiration.

I have worked with many patients with a nissen... and yes they still have problems... Sometimes very bad... Lots of mucous, (etc). gagging, wretching. Usally requires a lot of venting. These patients will usally receive night nursing so that they have an awake care provider. If they become ill, it can become worse (more secretions, etc.) It isn't fool proof either. The nissen can become loose. Good luck..... Take care..
