How does your OR handle hand off's for lunches, breaks, change of shift?

Other than a oral report and count is this documented anywhere for you or even your report to PACU?

We document breaks, lunches and change of shift in our nurses notes on the Op. record. We always do a count when staff changes as well.

Do you document the hand off during breaks, meaning what was told in the report?

We just document who gave the break and the times, for example: "Nurse so-and-so,RN, circ. relief 1200-1230". Nothing is specifically documented about report (a verbal report is given, of course).
