Can someone tell me what these abbreviations mean? Fundal height documentated c/w and s=d ? I am not an Ob nurse and reviewing a chart and just curious!

c/w is 'correlates with' in my experience.

I'm not sure about the other.

Not positive, but I'd guess "fundal height Consistent With" dates. And for the other one, possibly "size equals, or is appropriate for dates".
