Hey everyone,

I am in the middle of doing a care plan. I have a patient who does not have much to work with for my topic of the week. We are doing Socialization. Anyways, she is pretty much self care and lives in a nursing home. The only thing I can work with is that she has reading glasses. Are reading glasses considered a "Disturbed sensory perception"? Because the patient can see fine without glasses, its only when she reads that she needs them. So I cannot even start my care plan until I find out about the reading glasses.

Is she happy with her level of socialization? Does she want to get out more, and can she get to all the activities she wants to attend and participate without difficulty? What do you find in NANDA-I 2012-2014 that looks at roles, coping, life principles, comfort, safety? I'll bet if you look in the table of contents for each of those sections you'll find something.
