hi, i need one more nursing dx for a major care plan. anyone have any suggestions... it can't be r/t to nutrition... i have 5 already but need 6....any help is appreciated... i can not believe my nursing care plan books only have dx dealing with nutrition...

How about "Risk for Decreased Cardiac Output r/t increased afterload 2nd to HTN," or "Ineffective Therapeutic Regimen" or "Risk for Cardiac Injury (left ventricular hypertrophy) r/t increased afterload and strain on the heart 2nd to HTN"

What do you have so far?

"Risk for ineffective Tissue Perfusion (Cerebral) R/T interrruption of blood flow"

Risk for distributed sensory Perception (global) R/T neurological trauma second hypertension and occlusion of blood flow to brain tissues"

Risk for Ineffective Renal Tissue Perfusion R/T untreated or uncontrolled stage 3 hypertension (BP >= 200/120 mmHg)
