How about "Risk for Decreased Cardiac Output r/t increased afterload 2nd to HTN," or "Ineffective Therapeutic Regimen" or "Risk for Cardiac Injury (left ventricular hypertrophy) r/t increased afterload and strain on the heart 2nd to HTN"

My Nsg Dx book also has: ( these are really good Dx to do patient teaching )

Ineffective Health maintenance r/t deficient knowledge regarding treatment and control of disease process

Noncompliance r/t side effects of treatment
