Did she make any statements about feeding, or did you have any conversations about this? Did she say why she was alternating? Include any pertinent statements from the mom. Chances are she doesn't think the baby is getting enough because her milk hasn't yet come in, and that's where the deficient knowledge comes in. I wouldn't put "r/t importance of breastfeeding", but rather, something about newborn feeding. Do you have a NANDA book?

Hi all! I'm new to this so hopefully Im posting in the right place. Anyway, I'm trying to write a care plan for a new mom and need help with one diagnosis. The mother decided to breast feed, but was found to be alternating between breastfeeding as well as formula feeding. (the hospital is baby friendly so this is not encouraged) I originally wrote deficient knowledge related to the importance of breastfeeding as evidence by patient alternating between breast milk and formula but it just doesn't seem quite right. Please help if you have any suggestions!
