patient spitting out medication

After watching a patient who is aox4 pop her medication and drink some water, I come back see a senna sitting on her bedside table. I asked her how did she not take the pill and she said she fell asleep and decided not to take it. Another nurse heard her comment during report and I edited medication as not given and putting the patients comment. I am nervous about getting a write up over this . Has anyone else experienced this?

I don't see anything problematic for you in this scenario. What aspect of it are you concerned about? You edited the mar given new information that they resfused senna, and now the record reflects correctly what they did/didnt take.

The patient has the right to refuse. And it's just Senna. Who cares about Senna?
