Doesn't your facility have its own existing template?

Ours is pretty basic: Sections on purpose, responsibility, definitions, implementation, etc

Other than that we write our own text

we are a small home health agency and it seems someone deleted it. I thought someone might have one for word I could use quickly instead of constructing my own.

Hey, Sal. Seems you'll have to follow existing format... header, footer, etc.

Shouldn't be too tough to add a simple template, maybe a few columns, to that, right?
