The Agency for Healthcare Research and Quality (AHRQ) has released a new guide to help hospitals better prepare patients to take care of themselves at home after they leave the hospital. The guide is a fillable PDF that hospital staff complete and share with English or Spanish-speaking patients before they leave. At home, patients can reference it when they have questions about how to take care of themselves and take it with them to follow up appointments.

Although extensive, A Guide for When I Leave the Hospital is in an easy to use readable format for patients. Looks like a great resourse for those hospitals having Nurse Navigator programs. Karen
