Well Written Cover Letters

What are the important elements of a cover letter? How long should it be? Beyond basic language and grammar skills, how can I make myself stand out just by reading the cover letter?

Does anyone have a well-laid-out template of a nursing cover letter?

Thank you,
Wendy
