CGFNS (CES) Registration

Hello everyone!

I just want to ask a help from you guys! I am Philippine Nurse and currently processing my application for CGFNS. I am residing in Japan with my military husband, we got married in New Mexico and I have my original copy of our marriage certificate. I recently went to PI just to process my papers in PRC (to change my status as well of my PRC license from single to married) I tried to do that but the NSO don't have a copy yet of our Marriage cert. and in order for them to have a copy they want me to get an appointment in US embassy manila, which is a big hassle for me because I only had a week to stay in PI so I had no choice but to keep my single name for now, and soon go to the legal office in our base to ask about my issue. My question is, since I am going to use my single name in CGFNS too what should I put in my marital status? Single or Married?

and how can I change my name in CGFNS after submitting the application because I have my original copy of my marriage certificate here. Or should I just change it after passing the NCLEX? =)

Women can keep their maiden name ("single name") if they choose to do so. It will not affect any of your rights and this is acceptable in the US. And yes, tick married.
