Pretty specific question, hopefully someone can help me out. New NP, just about to start my first job. Incredibly excited! I'm attempting to apply for my DEA number and need a state controlled substance license to go with it. Being in Michigan, the LARA website led me to this application... https://www.michigan.gov/documents/lara/lara_pharm_cs_app_0314_450189_7.pdf however, there is no nurse practitioner option in the drop down list. There is a PA option, is that what I'm supposed to pick? Is there another application entirely? Hope someone can help me out. Thanks!

Hi there. If you haven't found your answer yet, currently in Michigan only PAs need the state controlled license to go with the DEA license. In that area of the DEA license application you just put put your NP license again. Before they send you your license they will send you a form that needs to be filled out by the physician you are working with. Or you can get it by calling or emailing the DEA.

Hi! I am in the EXACT same position as you- but i paid the $88.60 and sent in the controlled substance license just for it to be returned to me lol. So where it asks for a michigan substance license number you put your NP License number again? And they automatically know to send a sheet out for the physician to sign?

In Michigan, you never send in an application for a CSL. You only need a DEA. You operate under the CSL of your collaborator. The place of employment handles the details of where scripts are routed and how.
