Breast milk identification

I'm a nursing student working on a teaching project for NICU nurses. The problem in this particular NICU is compliance with the breast milk protocol, more specifically the identification steps. The policy states that the milk should be removed from the fridge, brought to the bedside and stickers on the BM and infant band should be compared. The breast milk is to be prepared at the bedside and the container/syringe should be labeled with the infant's sticker. Before administration the sticker on the syringe should again be checked against the infant's band.

In my reteaching of the protocol I would like to provide rationale for each one of these steps to reinforce the use of evidence based practice with these nurses. I'm having trouble finding literature on what other units have done, studies to support double-check identification, research on system flaws, etc. I was wondering if anyone has come across anything that they used to develop your unit policies that would be of any help (current policy does not have references) or have come across anything that would be beneficial. We have to write on the flowsheet MBM checked by 2 RN's and then we both initial it. I don't think that there has been research done, since MBM is a body fluid it is treated as such.
