Kaplan study question

I have been utilizing the Kaplan Qbank for studying for the NCLEX. I've come across a few SATA questions that I keep getting wrong. For example, one of the questions I got today was regarding discharge teaching for a chronic kidney failure patient. One of the answers was, " I should take acetaminophen instead of ibuprofen for a headache." Knowing that ibuprofen is nephrotoxic, i chose it as correct. However, Kaplan states in the rationale that, " the client should not take any over-the-counter medications without consulting the health care provider, since they may be toxic to the kidneys"

I've seen this rationale pop up quite frequently throughout my Kaplan Qbank questions. Is this information that I should go by when I take the Nclex? I know that many test-prep resources may differ on content.

Kaplan is imperfect. I don't agree with the rationale. However, the point they appear to be trying to make in this case is that all CKD medications should be discussed with a prescriber. While that is true, it may not be real world, since the nurse often screens. The takeaway is that CKD is different. However, I feel this is a poor rationale.

That's a frustrating question, because in real life that is probably something that we would teach patients. Perhaps what Kaplan is getting at is nursing scope of practice, since nurses techniquely can't prescribe medications. As such the patient is going home and probably needs labs reviewed with provider periodically before taking and over the counter meds. Don't stress too much, Kaplan prep is harder than the NCLEX.
