Can cgfns screw up in sending form to nursing college?

I had applied for the new york cgfns service, paid my fees, sent out forms and everything. T Now they have sent just my "authorization for verification of academic records/transcripts" form n when I contacted my college whether they will send my transcript and documents they said that there has to be another form besides that which the college has to fill and in which it is stated what documents are required. I am so confused. I finally thought after months of waiting it had arrived to my college and now they are telling me they need to have that form to do so.

can anyone tell me what is the form and is there a really a form as such?
