How do you keep track of your pt. info during the day. Do you carry the kardex's around or do you have a handy form you fill out? Please share!

I use our "action sheets" that are printed out for each patient. I write morning report notes in red. Things i need to give in report and new orders etc in black. I usually have them in my pocket throughout the day so i can pull them out and add to anything i need to give in report at shift end. Works good for me.

I am a new M/S nurse too! I love it! We have a sheet with name, vs, hx, dx, labs, iv sites & fluid, o2, allergies, diet, ect on it. We just fill in the blanks when we get report. I carry it around in my pocket & write anything that takes place during the day on it to pass on in report, or to ask doc's about. Its kinda like a mini pt chart...I would be lost without it. I can get up to 12 pts on it.....8 is the most I have had so far, & that is enough! I have been working in M/S for almost 6 wks. We still do hand charting so I also carry a clipboard & chart on each pt as I go.....keeps me caught up & when its time to go home I am done!

BTW...I should clarify I am a RNA (RN applicant) so not a real nurse yet! Waiting on my ATT!

I carry the pt kardexes (printed out from our computer) with me on my clipboard. I staple them all together and then write report in one corner of each kardex, and then write what I need to give report on in another corner -things like abnormal assessment findings, new orders, pain issues, plan of care, tele reading, labs, etc. It works really well for me.
