float nurse in LTC

I am a new grad and I am working as a float nurse in LTC. It is hard because I am new in this facility and don't know the residents and their preferences. They float me from one unit to another. i don't take care the same patients every day and don't know what their conditions were during previous shifts. I need to make decision if pt's condition changed or not during my shift. I am confused! Help!!!!!!!!

I love to hire new grads but not for per diem or float positions. It's not really a good way to get started, but......

If your facility uses a 24 hour report book, take a look at it when you first come in for your shift. That should clue you in to anything out of the ordinary with your residents. Pay attention to report from the out going nurse and most important listen to your CNAs. The good ones will spot a change in a resident long before you will since they spend so much more time with the residents. If a CNA tells you Mrs. Jones isn't acting right you need to go assess Mrs. Jones.

Love that! CNAs are the best. Ask them a lot of questions, cause they know the residents the best.
