Continuous Care

Does your hospice offer continuous care? If so, I am interested in knowing what situations you generally offer it for. Do you have a specific trigger list? How often is it used? Do you use prn staff or otherwise? What is your continuous care census compared to your general census?

Actually the hospice hubby is working with, director, is doing a capital campaign for an inpatient unit as they are having more and more of this type of client.

They use a mix of prn/hourly people both aides/lpns. No diagnosis trigger per se, just the increased need .

Do you do that where you work aimee?

We are working through the implementation process right now. Do you know the number of people in the prn pool your husband's hospice pulls from? Do you know how many people are on continuous care each day versus how many in the "regular census?"

Our hospice offers continuous care, for a variety of reasons....say if the pt is active and the cg is having a difficult time etc...it's really great for some families.

You know I will ask him when he gets home Aimee. Good questions. I know they are always looking for pool people.

They also use hospital/nursing home respite quite a bit.

Thanks, Renarian!

Mandee, do you have any idea how many people are on your census and how many you have on continuous care at any one time?

Census is around 90 and sometimes we have nobody on cc and other times we have one or two.

Aimee I just talked to hubby who is in a rural area hospice in Ohio and he said right now they have no one on continuous care. He said they have a very difficult time staffing it and it is the most highly scrutinized area as far as reviews.

Thanks, Mandy and Renerian!

Our hospice is in the process of building a CC Center . It will have 14 beds with area to expand. It will be staffed with RN's and Aides. With an on-call MD. It will be used by 16 counties. I'm afraid we will be full with in a matter of days when it opens. There is such a need. Our hospice has never offered CC and I always feel bad for the families that really need this service. I feel we leave them hanging out to day.

Mazzi good luck with your capital campaign for your inpatient unit. Do you know the architect name of the company doing it? Hubby is using Matrix?

Their unit will be small to start 6 beds.

That is great, Mazzi! Having our own hospice house is still a dream here but gradually the dream is changing from mere wisps of thought to something that has more potential to become a reality down the road.
