Recently our clerical/biller/coding expert took a job elsewhere and in the meantime, myself and a clerical person have been handed the job of assuring the correct codes are on 485's.

HELP!!! WHAT ARE THE CORRECT CODES AND CATEGORIES FOR DIABETES? ARE THE FOLLOWING CORRECT?

*DIAB, TYPE I (juvenile onset)-controlled 250.01

*DIAB, TYPE I (juvenile onset)-uncontrolled 250.03

*DIAB, TYPE II - controlled 250.00

*DIAB, TYPE II - uncontrolled 250.02

-Does it not matter that they are insulin dependent or not???

-If they are insulin dependent do we consider that uncontrolled?

-If they were uncontrolled while inpatient and placed on insulin or an oral antidiabetic and sugars are now within range do we chart controlled or uncontrolled?????

-Can a person be considered Type I diabetic if they had adult onset and are on insulin?

I need a refresher course in endocrinology!!!!!

Does anyone have Nancy Mcquire's HH Diagnosis Coding Answer Book? I keep getting e-mails and mailings advertising this book, it sounds like the answer to my prayers but it costs $349! I've been begging my supervisor, but she would like to see it's content before purchasing.

I have a 2001 ICD book and the front has lots of explanatory direction but none specific to HOME CARE.

p.s. Nurses in home care are such a rare and special breed. It's so nice to log on to this site and know that others are going through the same things. Even with all of the overwhelming requirements and red tape, I LOVE BEING A HOME CARE NURSE....(yeah, remind me I said this next monday when I want to quit)

This came out in Fall 01 ---think Nov/Dec as my Manager at the time had all the Intake/UR RN's complete as an inservice, corrected answer sheet with rationale why recomended codes used.

I learned a few things---NO ONE documents dominant/non-dominent hemiplegia in all my years (18) in homecare. Also, late effects of CVA per this article is to be used for clients if care rendered THREE MONTHS PAST CVA date---I had been coding that only after a year had gone by so agency missing out on additional dollars as some staff never even used that as a diagnosis but was a factor attecting pts ability to perform wd care, draw up insulin etc.

Ingrained in my RN education: learn one, do one, teach one, share the skill/knowledge learned.
