Charting at time of visit?

I am looking for some feedback regarding the specific issue of charting at the time of the home visit. Does your agency require documentation at the home or curbside? Our agency supervisors :angryfire dictate that all charting be done at time of visit and state that this is not just accepted practice, but "law." I was wondering about other agencies and your rules and timeline for charting completion.

We use paper charting and I have trigger finger in both of my thumbs so it's killer. I wish we would go to computer charting it's got to be faster (and less painful!). Since we use paper, the patient seems to appreciate charting in the home because we are writing down what she is reporting to us.

yep, especially when we're not getting paid to chart!!

i love charting. seriously. yeah, i'm a freakshow, but i really, really love the paperwork. ;)

I left homecare in 2003 after 13 years.

While you can complete your charting later, after you've left the home; the time of your visit starts when you enter the home & begin care-and ends when you've completed your actual physical visit.

If you do a lot of charting in the car, this may result in a short visit, and may affect whether it's a billable visit. Even if you took 2 hours to complete an admission & POT in the car, the actual billable visit is the time spent in the home.

So....try to get as much charted before leaving the home.
