I posted this question by accident on general nursing, doubt if it would be of interest there. My question is can anyone explain how they utilize the crosswalks with their MDS. If so, please inform. Again, I will state, hope this isn't a dumb ?....Tex

What is a crosswalk?

It is how you complete your MDS. For example if you mark something on the section G, modes of transfers using s/r's then when you mark under devices and restraints (section P) section. it affects your QI report. If you go to the HCFA site, put in crosswalks it will take you to a site with : MDS Version 2.0 and HCFA 672 + 802 forms. The surveyors use this. My ADON/Don and administrator are really trying to research this besides me. From what we know it affects your QI report. We are aware that we do a better job than what the report is stating. I also learned it could be our software. When we put in a dx for psychosis etc, when a resident is on a medication for this, our QI report says that we have someone on a psychotropic medication but no supporting diagnosis, when I know we have put it on the MDS and we have valid diagnosis to support it. This is why I presented the question, I don't know alot about it. Maybe some of the more experienced MDS coord. would know. I think it boils down to knowing how to fill your MDS out. QD I still learn and look at different sections and think why haven't I been doing this all along! I don't know if I gave the best explaination or the correct explaination to crosswalks, I welcome anyone to explain further. Tex

There are several web sites available on MDS information. Type minimum and data and set into your search section. Lots of good information on these sites. Pick the one that meets your needs the best.

I did many, many MDS and PPS, and never ran across the term "crosswalk." Thank goodness I'm now out of LTC.

Tex, in my past experiences with the QI & Behaviors sections... make sure you have documentation to back up what the QI flagged. IE... a resident with Organic Brain Disorder will flag out, however as long as you have the supporting diagnosis for the resident's meds, you will be ok with the surveyors. We had many residents from the state mental hospital, and our QI's flagged high in this area, however.... the diagnosis & documentation were there; they aren't accepted as such in the computer system. If you look in the RAI manual, you will find the diagnosis that are accepted. Just be sure that your charts are in order. It's all we can do. It's in the computer system. It won't print out right for you, even though you know you have the correct diagnosis for this resident. Do you have a checklist for the nurses to follow when residents are placed on psyc meds?

tex, there is a great tool that is part of your 672/802 instructions. It lists each QI and the crosswalks. I use it to work my RAPS. If you can't find it, let me know I will send it to you.
