Is this the norm?!?

I recently began a job in LTC as a new graduate, and am more than a bit concerned...I know LTC is demanding, but is it the norm to skip (what I consider to be essential) assessments, like vital signs? (I actually had a supervisor tell me to "Just make them up.":eek:) Additionally, I have witnessed meds, such as numerous anti-HTN's, given all in one med pass so the nurse did not have to do a second med pass, accuchecks skipped all together and MAR's falsified to reflect that the accuchecks were performed. Also, eye and ear drops documented as given, even though the bottles were still sealed...for a week! No one listens to bowel sounds when a patient c/o nausea, or takes a BP when a patient with known HTN has a headache -- the patient simply gets two tylenol, and is told, "This will help.."

When I questioned these practices, another nurse told me to "This is how we do it here. If you want to work here, you'll keep your mouth shut." Is this how it is, or am I simply in a rotten facility?

If I were you, I would quit this place and absolutely report them. What if those residents were your family or loved one?

Get the heck out. End of story.

I work mostly in the retierment part of LTC.

Falsifying MAR's - no way no way no way! - especially checking BG's. However if a rt c/o nausea I dont listen for bs, I give gravol and have the aide go keep and eye on them, and them call to find out how they are. Where I work its like they are in there home, if they are nauseated are they going to listen to bs, no. Again where I am is not acute, most are in semi-fair health.

But I agree with those who said run, when you are being told to lie in your charting, that is not a good sign

Rotten facility! but I don't think you need us to tell you that because you already knew. Trust your instincts. NO it is not the norm at all. Get out as soon as you can, Protect your license!

Thank you to all posted replies! It's good to know not all facilities operate this way. I took the advice of looking into the most recent state inspection of the facility -- it is operating on a "Provisional II" license. In addition, I am also taking the advice to get out of there...this job isn't worth my license. Thank you again!

Don't ever, ever, ever leave a job without a two week's notice. That will come back to haunt you later.

Not the norm, and another example of why LTC has the rep it has and cannot get/keep good RNs. Sad. The $$ wins again over quality pt care. Get out and save your license and reputation and report the facility!! Places like this need to be shut down and the pts moved to a properly run facility. LTC needs reform!

If YOU practice safe nursing; if YOU document only those things you've done and kept YOUR records clear, you will not lose your license. You can not be held responsible for what other people do. We are all responsible for our own practice. That being said, please tell the director of nursing what's going on...especially about being told to make up vital signs. She really might not know what's going on.

Well, I gave my two weeks notice! When asked, I told the DON about the things I'd seen in my time there...she was pretty upset. Apparently, she had no idea about some of the things going on there. The poor woman...she is also new to the facility. I don't envy the job ahead of her - getting that place back up to standard.

Still, I'm out of there. I'm new to nursing. I want to learn the right way to do things in the real world, although it may not be the easiest way. Thank you again, everyone, for the wonderful advice!
