Do all of your LTC charts have face sheets?

I can't believe this LTC FACILITY! I can say truthfully that approximately 30% of the charts have no face sheets. Nothing. No, I am not talking about the new admits either. There are residents who have been here a long time and still have no face sheet!!!!! The Lab gets mad at us for not putting the social security numbers on the lab sheets as requested. I would love to put the social security numbers on the lab sheets but I have no face sheet where the social security numbers are listed. So I am curious. Are other facilities also missing their face sheets too? Or are we just special?

The facility I work at uses computer charting but we also have hard charts that we file orders, labs, etc. in & we have face sheets for both! I can't imagine having quick access to that info, I would be terrified of making a mistake without proper identification...

We have face-sheets in our charts.

- Where do you keep their POC/guardianship info?

- What happens if you have to send them out asap? What do you give to emergency personnel?

An emergency is stressful enough w/o scrambling for info.

"Failure to notify family (or MD) of significant changes in a timely manner" is a cite-able deficiency from the DOH or survey team. You can bring this factoid up to your manglement as a precautionary possible accident waiting to happen. I said it earlier, I have had that problem with unstable, revolving-door new admissions. I know I'd be very PO'd if someone didn't call me as my kin was being sent out at the time.!

I have worked at facilities that have been nailed BIG TIME for that defieciency. So they are usually very gun-shy. Maybe that can help your case for updated face sheets.

Your facility does have its problems. Good luck.

We use a temporary face sheet the hospital sends us with all their information and our facility face sheet is on the chart the next working day of the admission.
