  1. Yes, I work at the "Anything Goes" LTC facility. The CNA's are in charge at this facility. The RN supervisors let them run the place. The RN supervisors are well aware of the missing face sheets. I was told by the other nurses that management is aware of the missing face sheets but still nothing is done about it. At the next staff meeting, I will ask about the "missing face sheets."

Ahh, but I didn't ask if you went to the RN supervisors. I asked if you had informed the DON or administrator. Don't wait for the next meeting.

  3. The facility I work at uses computer charting but we also have hard charts that we file orders, labs, etc. in & we have face sheets for both! I can't imagine having quick access to that info, I would be terrified of making a mistake without proper identification...

  4. We have face-sheets in our charts.

    - Where do you keep their POC/guardianship info?
    - What happens if you have to send them out asap? What do you give to emergency personnel?
An emergency is stressful enough w/o scrambling for info.

  5. "Failure to notify family (or MD) of significant changes in a timely manner" is a cite-able deficiency from the DOH or survey team. You can bring this factoid up to your manglement as a precautionary possible accident waiting to happen. I said it earlier, I have had that problem with unstable, revolving-door new admissions. I know I'd be very PO'd if someone didn't call me as my kin was being sent out at the time.!

I have worked at facilities that have been nailed BIG TIME for that defieciency. So they are usually very gun-shy. Maybe that can help your case for updated face sheets.

Your facility does have its problems. Good luck.
