Does anyone know of a Good NSG Diagnosis for someone with Rheumotoid Arthritis and a good Care Plan for it also.. I have a pt who has it, and it pretty much doesn't allow her to do much..

what about Activity Intolerance? thats the first one that came to my mind. if i think of any more, i will post

Look at how much it affects her social life, how she handles household tasks. Maybe a coping dx, or a self care deficit R/t immobility and pain.

Risk of Injury R/T reduced mobility AEB .......you gotta decide on this one (-if your professors require this step of course)

Let's see here there is a possibility for 1) Fatigue r/t chronic inflammatory disease, 2)pain r/t swollen or inflammed joints, 3)risk for injury r/t impaired physical mobility, 4)impaired physical mobility r/t pain or restricted joint movement, 5)activity intolerance r/t restricted joint movement or weakness, 6)altered health maintenance r/t knowledge deficit regarding care of arthritis. That is all I can think of for now.

Possible Goals
Will be free of S/S of pain and discomfort as evidenced by [ SPECIFY ] x 30 days.
Will be comfortable as evidenced by no guarding of [ AREA OF PAIN ] x 30 days.
Will be comfortable as evidenced by no moaning or restlessness x 30 days.
Possible Approaches
Administer medication as ordered for pain relief [specify].
Encourage and promote exercise within degree of toleration to promote pain relief.
Teach resident relaxation techniques and diversional therapy as alternative methods of pain management.
Assess characteristics of pain (location, duration, quality, aggravating/ alleviating factors, radiation, intensity) & DOC.
Assess effectiveness of pain medication.
Monitor & Document factors affecting pain (e.g. sleep, family relations, emotions)
Reinforce positive pain coping behavior.
Administer hot or cold compresses as ordered to promote pain relief.

If activity intolerence is not where you want to go chronic pain RT arthritis AMB (add behaviors such as guarding, atrophy ext here). Good luck!

Betts.. Thanks for all the goals and stuff.... Those are actually the things that I have more problems with... Once I get the Diagnosis, trying to figure out "individualized" goal and outcomes is the pain I have... But yours sound great..
