" Brain" sheets

Next week I will be team leading for a group of 4-6 patients. Does anyone have a template of a " brain sheet" that they use to organize all of the pt information? We need to have it one one sheet. I don't need to formulate my own. If anyone has one to share, or ideas on how to organize my sheet I'd appreciate it. Thanks! :)

thanks so much for posting this link, I used this 'brain' at work this weekend and it truly helped me keep organized but mostily helped me with the end of shift report. Thanks again!
