Hello All,

I am a last semester student at a local college for my BSN. We have to do a quality improvement project and was just curious what some local hospitals are working on changing in the area? What issues within your organization do you feel need attention?

Including more evidence based practice research into existing guidelines, etc.

Better flow of information from dept to dept, whether it be SBAR or which dept is supposed to take care of/initiate or discontinue what orders

Increase pt satisfaction scores

Revamp old policy and procedures

Get the voice of the employees via survey on what areas need additional improvement

We are currently revising our medication administration policy, wound protocols and falls protocol across the organization. The goal is to improve patient safety.

How are you revising your falls protocol? Or tell me a little more about your falls protocol....

A committee is discussing the current falls policy, signage and the documentation tools we currently have in place. All of the policies/ documents will be revised to make them more comprehensive and user friendly.

The drafts have been distributed to other members of the organization for feedback, including the health care aides.

Conducting thorough and complete head to toe assessments so that other protocol to the benefit of the patient can be instituted. Fall, skin, discharge planning....not to mention PT/OT/Speech swallow...

Check with the Risk Manager at a local facility. They are CONSTANTLY finding problems that need to be addressed. Sometimes a perceived problem may not be what it seems when someone can devote time. My problem turned out to be nothing that the hospital itself or the physicians were doing wrong...just many of the units and clinics were still using OLD orders! Promptly fixed!
