I was wondering if any nurses out there would be willing to sharea copy of their report sheets?

I have a couple from the nurses at my clinical sites and I am trying to compile some to get one that fits my personal needs. I have read several threads and some people have described theirs, but is difficult to follow sometimes.

We have a report sheet that we sent to floors we're sending patients to. We include:

Dx, Admitting MD and pager #, history of present illness/pmhx; Allergies; meds patient has taken while in ER; Cardiac rhythm; Abnormal lab values; IV info- size, location, start date, fluids; mental status (A&O?), I/O's; foley/urinal; most recent VS and admit to ER VS; Any labs/x-rays that are in need or in process;

If this is for a floor (shift to shift) I'd add: diet, activity level, FSBS freq and most recent

HTH- All of this info fits onto one normal sized piece of paper!
