Patient charts - advice for Czech republic

We are trying to develop standards for patient charts (cahrtholders) in our country. We know only two types, one that is common in North America (ring binder with color dividers) and one that is used in some European countries (HINZ system). We would like to hear about your idea about the most practical chartbinder. At this point, we are not so worried about specific forms.

Nurses from European and other countries - help.

Ahoj Magdo!

I saw the Czech part and had to see what was up. I am in Canada and here we have one box of cards with basic patient info (like diagnosis, meds...) so we can catch up, and each patient has there own binder with coloured dividers which have a complete history and more in depth info. It seems to work well enough.
