Need a CNA/HHA competency test form

We are starting up a new hospice and I need to set up a good screening and documentation system for new employees.

I found a HHA/CNA competency checklist that will work, but I still need the actual multiple choice HHA/CNA competency test.

Anyone have one they are willing to share?

Sorry, I do not have one for you.... we always utilized hands on demonstration of skills and discussed rational, etc during the "competency". Have you spoken with any of the local training agencies? They could perhaps assist you to develop one. I doubt they have one prepared that is comprehensive of all the skills that you may need.
