I did an admission( hospice) on my second day of employment. I know, ridiculous. I had no training on their EMR. the nurse manager entered the admission and assessment, signed herself as admission nurse. she never laid eyes on the pt. seems like fraud to me, what do you think?

No, it's not fraud. Fraud is deception for financial gain. But it's probably against hospital policy. Did you tell her what you found on your assessment, and she charted that? If so, a simple addendum stating that you completed the assessment and that she is charting per your report would make it legit such as "assessment per Cheryl Jones RN report." But nurses charting based on new orientee reports is certainly not uncommon.

This is falsifying documentation which is fraud. Hospice gets paid based on the admission assessment per the RN and their admitting qualification criteria. Since this said nurse did not partake in the actual assessment it is fraud and she is falsifying documentation.

You can perform and chart your assessment and she can go in and RN verify, meaning she can double check and oversee the documentation is correct. In no way should another nurse chart and assessment they did not do.

Thank your rkitty....I felt this was correct. I left this job after 5 days. This was the most glaring of numerous problems I saw
