I need some help with care plan on hypertension and diabetes mellitus.I do need 10 nursing interventions and their rationales plus evaluations on each.2 goals short term and long term and (expected outcomes). and 3 nursing diagnoses on each and should be in priority form. Thank you .

I recommended using the "Nursing Diagnosis Handbook" by Ackley/Ladwig. It was a lifesaver for those care plans for me.

ie pt with asthma. Whats an asthma attack look like? Bronchoconstriction. Translate to nursg. dx: Ineffective airway clearance RT bronchoconstriction AMB wheezing upon inspiration
So what does HTN do to a person? What causes HTN? What are the S/S? Figure that out and you can put it into care plan format. After you have your diagnosis, then you can start on goal and interventions. How are you going to take care of this pt? What are you assessing? How often? Acceptable paramaters? Tmts? Meds? Labs? Just make sure not to use any medical diagnosis.
You need to do your careplans yourself- they are an invaluable learning tool for figuring out a general map of how to take care of pts with those outlined problems.Last edit by beachbum3 on Feb 4, '08 - Feb 4, '08 by DaytoniteHi, smart girl, and welcome to allnurses!

- a physical examination of the patient
- observations you made of the patient while performing care
- a thorough reading of the patient's medical record
- looking up information about hypertension and diabetes mellitus (signs and symptoms, pathophysiology, drugs, therapies and treatments that the doctor orders to treat the symptoms)
Now, if you are having trouble with getting started with this, I will help you, but you need to start by posting the patient's symptoms. From there I will show you how nursing diagnoses are selected.suzi-Q likes this.
