    hi,
    I need some help with care plan on hypertension and diabetes mellitus.I do need 10 nursing interventions and their rationales plus evaluations on each.2 goals short term and long term and (expected outcomes). and 3 nursing diagnoses on each and should be in priority form. Thank you .

    I recommended using the "Nursing Diagnosis Handbook" by Ackley/Ladwig. It was a lifesaver for those care plans for me.

    As a start I try to think "what does this look like"

    ie pt with asthma. Whats an asthma attack look like? Bronchoconstriction. Translate to nursg. dx: Ineffective airway clearance RT bronchoconstriction AMB wheezing upon inspiration


    So what does HTN do to a person? What causes HTN? What are the S/S? Figure that out and you can put it into care plan format. After you have your diagnosis, then you can start on goal and interventions. How are you going to take care of this pt? What are you assessing? How often? Acceptable paramaters? Tmts? Meds? Labs? Just make sure not to use any medical diagnosis.

    You need to do your careplans yourself- they are an invaluable learning tool for figuring out a general map of how to take care of pts with those outlined problems.

    You should also read these latest threads on how to write a care plan so I don't need to repeat myself:To sum up what is on those two threads above, you can do nothing until you complete your assessment activity. The foundation of any care plan is the patient's signs and symptoms of their response to the medical disease or condition. This patient's hypertension and diabetes are only important for the signs and symptoms that the patient may have as a result of them, and for the pathophysiology of them that may or may not contribute to the nursing problems you will discover. Your nursing diagnoses, goals, and nursing interventions all stem from the patient's signs and symptoms. And, since they are directly extracted from your assessment (they are the abnormal data that you collect) you cannot proceed with your care plan until you have completed your assessment. Assessment includes:
    1. a physical examination of the patient
    2. observations you made of the patient while performing care
    3. a thorough reading of the patient's medical record
    4. looking up information about hypertension and diabetes mellitus (signs and symptoms, pathophysiology, drugs, therapies and treatments that the doctor orders to treat the symptoms)
    Only then, can you make an all-inclusive list of the patient's symptoms. It is from this list that you choose your nursing diagnoses, develop goals and nursing interventions. Goals and nursing interventions are ALWAYS addressing the patient's symptoms.

    Now, if you are having trouble with getting started with this, I will help you, but you need to start by posting the patient's symptoms. From there I will show you how nursing diagnoses are selected.
