Psych evals from nursing homes

Had a pt come from a facility for med clearance for a psych eval.

Pt was basically, just a jerk. Can't pink slip someone for being a jerk. Especially if they are a-o x's 4. Not threatening to hurt himself or others.

The facility nurse was beyond rude with me when I called and said they were returning. I think they were just trying to find a reason to have him admitted so they thought they could do some magic and refuse to readmit him.

We all know less than friendly, jerk pts. I just couldn't explain to the staff, including the management there, why we can't pink slip a person just because they do some rude things. Especially when they aren't a ltc pt, but rather just there for therapy after an incident.

For a chuckle.. When giving report, the nurse told me 'well, THATS why he needs a psych eval, there's only A-O times 3 and you said he's 4'

We see these often. Near as i can figure out, the problem is a personality conflict between the pt & the staff. Most times, it's med eval then psych eval then back to facility.
