Evidence of swine flu risk to pregnant women rises; experts urge early treatment

Remember that the CDC is now recommending special considerations for pregnant HCW. Most hospitals and health care facilities have not yet implemented those guidelines. They might not if no one mentions this to Risk Management. Feel free to copy this article and show it to RM with the CDC guidelines. I am going to be doing this also.

If you are pregnant and you get sick, take the Tamiflu. It is safer than not taking it.

There are mounting and troubling signs that swine flu and pregnancy don't mix well.

Six pregnant women in Manitoba are reportedly on ventilators because they are severely ill with the virus.

And at least two pregnant women in the United States have died of swine flu complications after delivering babies by C-section.

A pregnant teenager in the Dominican Republic died, as did a pregnant woman in Scotland.

A woman in St. Theresa Point, a First Nations community in Manitoba, miscarried after contracting swine flu.

Humankind's relationship with the new swine H1N1 virus is still in its infancy. But people who've studied the issue of pregnancy during flu pandemics don't like the signs they are seeing. Dr. Denise Jamieson, an obstetrician-gynecologist with the U.S. Centers for Disease Control's division of reproductive health, says she finds the evidence to date "very unsettling."

"I am concerned about this," Jamieson said in an interview from Atlanta.

"There does seem to be increased severity in pregnancy. We don't have hard and fast numbers but there are enough reports that are concerning."

Data released by the CDC last month said at that point, 17 per cent of Americans hospitalized for severe swine flu infections were pregnant women.

A report a couple of weeks back in the World Health Organization's journal, Weekly Epidemiologic Record, noted of 30 swine flu patients hospitalized in California, five were pregnant women. Of those, two developed severe complications - spontaneous abortion and premature rupture of membranes.

...the fatality rate was higher in pregnant women during the 1918 and 1957 pandemics, though not the milder pandemic of 1968.

"If we base it on what we know of the 1918, 1957 pandemics, what we know about pre-existing antibody levels to swine influenza in the population, based on that I would say for this particular virus, pregnant women may suffer more serious consequences, especially in the third trimester," she said.

"And they should probably seek care early if they have influenza-like illness."

Studies done after the disastrous 1918 Spanish flu - which took its heaviest toll on young adults - showed astonishing death rates among pregnant women, said Dr. Michael Osterholm, an infectious diseases expert at the University of Minnesota.

Skowronski's review paper suggests there were also very high rates of spontaneous abortions during that pandemic - 26 per cent in pregnant women who became infected and 52 per cent among those who went on to develop pneumonia from their infection.

Osterholm explained pregnancy is a precarious state for a woman from an immunological point of view. In order that the mother's body does not reject the fetus, part of the immune system has to be effectively dialled down.

Other factors are also believed to come into play, including reduced lung capacity, Jamieson added.

She said that while the CDC doesn't yet have firm numbers, they are hearing that some pregnant women are reluctant to take antiviral drugs when they are diagnosed with swine flu. In some cases, their physicians share the reluctance.

Jamieson said given the risk swine flu poses to pregnant women, any who feel they may have contracted it should seek care quickly and should tell their doctor about potential exposures to people who had the virus. And they should take the antiviral drugs, she said.

"The message we're trying to get out is: 'Don't delay. If you suspect influenza, initiate antiviral therapy appropriately even before you get the testing back," Jamieson said.

"We definitely feel like in a situation like this, the benefits outweigh the risks of giving antiviral medication."

Swine flu has appeared among Venezuela's Yanomami Indians, one of the largest isolated indigenous groups in the Amazon, and a doctor said Wednesday that the virus is suspected in seven deaths, including six infants.

The deaths happened in forest villages near Venezuela's border with Brazil over the past 2 1/2 weeks, said Raidan Bernade, a Venezuelan doctor on a team working to contain the outbreak and treat the ill.

Bernade told The Associated Press that doctors confirmed one of those who died had swine flu-a 35-year-old Yanomami woman who doctors believe was pregnant.

Health department spokesman Larry Vasko told FOX Toledo News that Mandy Kimbler died Thursday at Toledo Hospital. She had given birth to twin boys in the past week.

Vasko said the cause of Ms. Kimbler's death is "suspected to be complications from H1N1" or Swine Flu. She was admitted to Toledo Hospital in late October for flu-like symptoms. She delivered her sons prematurely last week. According to her Facebook page they were not due until early January.

Samples have been sent to a lab for analysis, but it will be a few more days until county health officials obtain the results and can confirm she was infected with Swine flu.

To his knowledge Ms. Kimbler had no other underlying illness and was not vaccinated against H1N1 Dr. Grossman said.

Peshawar, Pakistan

An Afghan female who was infected with A/H1N1 influenza died at a hospital in northwest Pakistan's city of Peshawar on Saturday, hospital authorities confirmed.

Hospital authorities informed the official news agency APP that Lateefa Bibi, 40, from Helmand Province of Afghanistan was suffering from high fever for the last few days. She was admitted in North West Hospital in Hayatabad, a modern suburb on the southwestern fringe of Peshawar, where her blood samples were obtained for laboratory tests by the World Health Organization (WHO) team who later confirmed the A/H1N1 influenza.

After confirmation of the A/H1N1 influenza in the Afghan female, the hospital authorities also obtained the blood samples of the newborn baby of the infected female, her attendants and the staff of the hospital, the hospital authorities said.

Sharqia Governate, Egypt

Egypt has registered the sixth A/H1N1 flu death case in the country, Al-Ahram daily reported on Wednesday.

A 24-year-old pregnant woman from Sharqia governorate died of A/H1N1 virus in the hospital on Tuesday night, the report said.

The woman returned from Italy on Oct. 25 and was hospitalized on Nov. 5, according to the report.

It sounds like the rapid flu test was negative like so many others. Now, she will be diagnosed at autopsy.

The 33-year-old from Germany was not suffering from any other pre-existing health conditions when she was taken to the hospital with flu-like symptoms.

An initial test for swine flu had turned up negative, but as her conditioned worsened she prematurely gave birth even though she was only in her seventh month of pregnancy.

The woman was then brought to the University Clinic in Aachen where she died of lung and circulatory failure despite being put on a respirator.

this is what is happening in china, to stop the spread of h1n1. they say it may have worked....... if it did, you can expect more mandatory vaccination happening here, schools closing, businesses closed, and no movie theaters open, no trips to the supermarket, or travel anywhere!

if doing that could save the lives of one mother and baby, would all that be worth it to you?

it would be to me. you can't morally put a price on lives, especially those of unborn babies, so get the vaccine, folks, before you have the one for seasonal flu, as it's been shown statistically in canada, with 2,000 people that h1n1 flu frequently occurs after someone gets the seasonal flu vaccine. there's plenty of time before seasonal flu gets here in late dec., jan., feb., and march. i got the seasonal flu vaccine at the beginning of september, and then got what appeared to be h1n1 flu a month or so later - for the second time. anecdotal i know, and the whole story of the statistics being assembled now, will be known next year.

A pregnant woman from the Mekong Delta province of Kien Giang, six months into her term, has become the 41st victim of swine flu, a health official said November 11.

Dr. Nguyen Huy Nga, chief of the Preventive Health and Environment Department, said at a meeting of the National Steering Board of Human Flu Prevention that the 16-year-old developed a fever, cough, and sore throat on November 3 and bought medicines to treat herself.

Four days later, when her condition worsened, she was taken to the province's General Hospital where she died on November 9.

This is something that will present greater impact on people vulnerable to these infectious organisms, hospitals and care providers, making the situation today with H1N1 look like less than the tip of an iceberg.

We need to put aside the resistence to being told to do it and avail ourselves of vaccination against H1N1, then seasonal flu; and promote it for others, as we never have before. That could prevent greater loss of life that will happen, when/if H5N1 becomes entwined with H1N1. Fatalities with H5N1 are much higher than H1N1 in the countries were it's prevalent, especially for very young children.

I've never been a fear monger before, but the feeling that this time is precious for prevention of contagious type A flus has never been greater, for me. We need also to avoid crowded situations, have enough chicken soup/comfort food, jello, fruit, and juice in the freezer for your whole families, etc so we don't have to shop while ill.

More markets and pharmacies need to deliver medications and food to homes, so those infected will stay there, and of course have the TV servers and other utilities paid up in preparation for the possibility of having to stay home.

It's unfortunate that so many "backs got up" when officious administrators put forth mandatory declarations. That aside now, we need to face facts and protect ourselves and others! New findings show that the highest fatality rate due to H1N1, is seniors. That group hasn't demonstrated the anticipated protection through earlier exposure to it.

Don't know if that will ever come about. The quote is from the Vietnamese. Clearly

all of the countries where H5N1 is endemic are worried about that possibility as

are some virolgists in the western world also, although we don't hear it spoken of as

much.

I know they are concerned in Hong Kong, and the Egyptians slaughtered all those

pigs not just because of swine flu. Margaret Chan of the WHO had also said that no

one knows how H5N1 will behave under pressure of a pandemic virus. Things could

get more interesting but we already have enough on our plates...

Yes, vaccination is a very wise idea for even if the virus mutates, which of course it

will, it may offer protection against a fatal outcome even if infected.

An Egyptian female dies of swine flu, raising number of fatalities from A(H1N1) virus to eight, health ministry spokesman Abdulrahamn Shaheen said Sunday.

He said the 26-year-old female from Alexandria was admitted into the infectious diseases hospital in the Mediterranean city yesterday suffering from fever, coughing and breathing difficulties.

He said the female, whose name was not disclosed, was pregnant in her ninth month and her health condition was critical.

This pregnant teacher survived.

On Halloween, Johnson found herself severely short of breath. Her oxygen level was at about 70 percent of what it should have been. Chest X-rays showed her lungs full of fluid.

By the next morning, Johnson was breathing with an oxygen mask in Methodist's critical-care unit, where the worst H1N1 patients come. Soon she needed a ventilator, a feeding tube, an IV in her neck. Doctors said Johnson's chances at survival were a tossup, and they constantly monitored her baby.

On Friday afternoon, Johnson was discharged. Her baby is due in January. Her doctor told her to stay home a week before returning to her job as a first-grade teacher in Newton. But Johnson wasn't thinking of that. She was thinking of finally going home and seeing her dog, a puggle named Harlie.
