lost the hard copy of my CNA license!!

How can I replace it? I know my number by heart, but some employers want copies of the license itself. I just moved and it must have got lost in the move when organizing (silly I know)

How would I get another copy? How much does it cost? I live in MA by the way

Get the address of the agency that issued it and write them a letter requesting a replacement. If you know the fee, send it. They probably need your signature to do this transaction. You should get the info online. The procedure may be online; perhaps you can request it online without a signature. Or you can call, to find out how to do it.
