I'm doing my OB rotation and working on my care plan. The newborn I'm working with was exposed to the Zika Virus and is staying longer in the hospital, while mom is being discharged. The baby does not have microcephaly, but they are concerned about a reddened area around the umbilicus and CMV coming back positive and >8.0.
I'm looking for some advice on the nursing diagnoses I have come up with so far and any suggestions.
Altered Family Process r/t prolonged hospitalization and separation from family
Knowledge deficit r/t zika virus aeb language barrier
Impaired skin integrity r/t attached umbilical cord aeb reddened area around umbilicus
Risk for infection r/t exposure to zika virus aeb reddened area around umbilicus
Risk for sudden infant death syndrome r/t exposure to zika virus

What have you assessed in the baby? Nursing dx are made based upon assessment, not upon medical dx.
I will say that language barrier is not an appropriate manifestation. Questions like "Zika virus? What is that?" would demonstrate a knowledge deficit. But if this mom isn't English speaking, the use of certified medical interpreters is required, eliminating the language barrier. Also, the medical dx isn't the r/t. If there is a knowledge deficit, it would be "knowledge deficit: zika virus, r/t *cause* amb *reason why you know there is a knowledge deficit*." Zika virus doesn't *cause* the knowledge deficit.
Do you have a NANDA-I book? If not, get one. It's an excellent resource for nursing dx.
