One of my homework assignments is to complete an assessment on someone of my choice. They have to sign a release for me to do it, but I was wondering about the "name" in the paper; do I use initials only?

This isn't an APA question (although the OWL links are excellent for APA information).
Yes, you can use initials, or better, a pseudonym. Release or no, be sure to disguise any identifying information (change age, employer and type of work, city, etc.) so no one can read it and connect it back to a real person.
Also, unless you are doing profiles on more than one person, "he" or "she" signs your release, not "they."
