Work Injuries, patient care

This is kinda of lengthy, but I will try and make it as short as possible and still give enough info. Wondering if anyone can shed light on this or if they have had a similar experience.... I would really appreciate any input.

I am a nurse, and got a needle stick injury a few months ago, then the recently got hurt repositioning a patient.

Hurt my back repositioning a patient in bed with help. Received in report that patient was a SBA, when in fact they were a max assist. I had also received in report that I only had one total care that night, when in fact there were three on my assignment. When I brought this to the attention of the charge nurse, I was told they were not listed as total cares. The same assignment was split up earlier that day, and then again for the next day, and the patients were changed to total cares.

Okay, so this is now the third time this has happened to me where the assignment was split up on days, then all given to me at night, split up again on days, and with the assignment having patients that are total cares not listed as such. This has also happened to me with confused patients several times in different hallways, and then split up again on days or the next night. This has been an ongoing problem since I have worked there, and I have only been there about 8 months. I have brought this to the attention of the educator, director, charge nurses, and clinical managers. They have all agreed, but nothing has changed.

I was then told I need remediation. I now feel I am being blamed, and I have repeatedly brought this issue up again and again to people, but am not getting any support. Any ideas?
