Just recently I had a note I had written on a patient striked out without my permission by corporate, It had my name attached to it as though I was the one who did it and done on a day I was not at work. The Directer of Nursing told me because I was the one that originally created the note my name is attached to it. It was on a patient the state is involved with. Was told I should have documented in another part of chart where state can't see it, only we can see it. I was told coorporate has athority to do this.
Generally only the writer can strike out their own note, but I'm guessing that there was some data you included that should have been on an incident report (which are not accessible to the state) and not directly in the notes.
