Where can I find my PRC Certificate Number?

I am a june 2009 board passer and got my license in august last year. Unfortunately, I lost my wallet together with all my ID's in it including my PRC license. I'm working on getting a replacement for my lost prc card so I downloaded the Action Sheet for the said replacement. However, it's asking for a certificate number which I couldn't find on my board certification form. I really need my license. Help anyone?

I think you are on the wrong forum. This should be moved to the Philippines forum if only I know how. LOL... From what I remembered, the certificate number is the same as the Registration Number. Good Luck on getting a replacement card! I heard it takes a while.. Better have your ID duplicated, and carry the copy with you in your wallet and leave the original at home.

go to PRC immediately or call.....declare ur ID as lost.ask wat r the steps or requirements needed 2 get a new ID..coz it will take long months to get a new one...
