I was looking at a website and reading job descriptions for the NP and it had this listed: "practice in the case management model of care." Any idea what that means? I assume that it is collabrative as this is an academic center, but I don't really know much more. The rest of the description leads me to beleive that the NPs carry their own patients (ie. not just cover residents and assist...) but I was hoping some one could shed some light on that.

Case management assigns the administration of care for a patient to a single person (or team); this includes coordinating all necessary health care, along with required supportive services.
