How would you deal with someone (a co-worker) you observed working outside their scope of practice?

My first step would be to address the issue with the individual, in a questioning rather than accusatory way -- "Gee, I notice you're doing X; I've always heard that RNs aren't allowed to do that without an order (or whatever the concern is); were you taught something different?" Sometimes it's just a simple misunderstanding or misinformation that can be easily cleared up. If that doesn't resolve the issue, the next step would be to go to the person's supervisor and share your concerns. If the responsible nursing supervisory/administrative personnel don't mind people practicing outside their scope of practice, the next step (if you choose to take it that far) would be the BON.

I would tread carefully, though, and be v. sure that your information is correct before challenging others' practice. This is a difficult type of situation -- best wishes!

Personally I would call them on it... If they are working outside their scope I would let them know if it doesn't stop... I have no choice but to report it. You may ask why I don't just report it instead of confronting them... I will tell you why. When a nurse is turned in the the nursing commission they have to investigate the aledged act of misconduct. Problem is... if there is no misconduct the fact that they were investigated will haunt them for the rest of their career. That red flag remains on their record justified or not.. I know... I was falsely accused. Perhaps your co-worker is just to dense to realise that what they are doing is putting both of you in jepardy... I say give em a chance to stop it... on the other hand, If you have noticed what they are doing... its very likely that others have too.. they may already be turned in.. which if you do confront them you will be sure to want to mention.
