What does a LVN Case Manager do?

Would anyone be willing to share their knowledge about what a LVN Case Manager does? I really want to get out of a skilled nursing facility and would like to consider being a Case Manager.

hi, im an LPN and work in Case Management here in VA at a psych hospital. The job mostly consists of setting patients up with after care with Therapist and Psychiatrist and referring them to different resources in the community depending on their need. Since I am not a LCSW, I am not able to complete the initial assessment on the patients but do just about everything else. Hope that helps.
