What does HONDA stand for?

I overheard some nurses talking about HONDA patients. I understand that it is what they call the most common cluster of conditions patients have. I heard part of what it stands for but I can't figure out the N.

H= hypertension

O= obesity

N= ????

D= Diabetes

A= Asthma

Anyone know or guess what N is?
