What court records does the Oklahoma BON require when applying for boards?

I have a defered class c misdemeanor for consumption of alcohol by a minor on my record. I was only given a ticket and paid my fines and took care of all other requirements. No arrest and no court dates. What paperwork will I need to obtain to send to the BON when I apply to take boards?
